Arizona Physician Offices


During the current Centers for Medicare & Medicaid Services (CMS) Scope of Work, HSAG will work with a select group of Arizona physician offices with existing CCHIT-certified electronic health records to improve the quality and frequency of preventive health care services in order to optimize beneficiary
quality of life and health care efficiencies.


HSAG will work with these physician offices to implement care management processes to improve:

• Colorectal cancer (CRC) screening rates.
  
  
• Breast cancer screening rates.
  
  
• Influenza and pneumococcal immunization rates.

HSAG will also work with providers in this setting to review beneficiary complaints and discharge appeals and improve drug safety.
