Due to security requirements, on October 10, 2016, all dialysis data website Master Account Holder (MAH) passwords were reset, and all user accounts were disabled. New MAH passwords will be sent to facilities via email during the week of October 10th. Facility MAHs will need to log into their master account to re-enable facility user accounts and grant permissions prior to the start of the Preview Period (November 1st 2016).
