2) Enroll in NHSN based on dialysis facility type: Outpatient Dialysis or
Home Dialysis.
- Refer to the NHSN Enrollment Guidance or NHSN Facility Administrator Enrollment Guide March 2018 and the New User Checklist (Outpatient Dialysis or Home Dialysis) for guidance.
- If you receive a notice that the facility's Centers for Medicare & Medicaid Services (CMS) Certification Number (CCN) will not validate, please contact NHSNEnrollment@cdc.gov and request a temporary enrollment number (CDC Registration ID) to complete the process.
