On April 15, 2008, CMS published updated ESRD Conditions for Coverage (CfCs). CMS develops CfCs that healthcare organizations must meet in order to begin and continue participating in the Medicare and Medicaid programs.

These minimum health and safety standards are the foundation for improving quality and protecting the health and safety of beneficiaries.

Because regulations and documentation for the CfCs are frequently revised and updated, please visit the CMS website below for the most comprehensive and up-to-date CfC resources which includes but is not limited to:

  • ESRD Conditions-for-Coverage FAQs
  • Current Measures Assessment Tool (MAT)
  • ESRD Facility V-Tags & Identifiers
  • Core-Survey-Process Information
  • Infection-Control, Water-System, and Personnel Requirements
  • Components of Immediate Jeopardy/Guidelines for Citations
