The purpose of the Behavioral Health Readmission Audit Tool is to provide a structured tool for use in performing real-time audits on readmissions which occur within 30 days of an inpatient discharge. The goal is to help identify actionable interventions to improve patient outcomes and decrease the likelihood of subsequent readmissions.
