According to SAMHSA (Substance Abuse and Mental Health Services Administration), people with mental and substance abuse disorders may die decades earlier than the average person, mostly from untreated and preventable chronic illnesses like hypertension, diabetes, obesity, and cardiovascular disease. Primary care settings are a gateway to the behavioral health system, and primary care providers increasingly need support and resources to screen and treat individuals with behavioral and general healthcare needs. The solution lies in integrated care.
Kristen Palcisco, CNP from MetroHealth, a Level 3-NCQA-recognized Patient-Centered Medical Home in Cleveland, will present on her experiences in integrating behavioral healthcare into a primary care setting. Her practice worked to implement depression screening throughout primary care, while a behavioral health model was being integrated. Ms. Palcisco will share the hesitation that primary care providers had when this model was being developed, and how her organization has overcome those challenges over the last several years.
