HSAG Vaccine Assistance Program is scheduled to end on October 31, 2024. Requests submitted after October 28, 2024, will be processed on a case-by-case basis as our ability to schedule vaccine clinic support is dependent on pharmacy availability. We encourage you to schedule as early as possible for a fall 2024 clinic.

Until October 31, 2024, HSAG will continue to assist with improving vaccine rates in the nursing home setting by:

  • Providing on site and virtual vaccine education to residents, family members, and staff
  • Scheduling vaccine clinics
    • SNFs must have a minimum need of 20 total vaccines. The 20 total includes Pneumonia, Flu (2024/2025 dose available September 15, 2024) and COVID-19; for both the staff member and resident populations.
    • If your facility is unable to meet the minimum of 20 vaccines, please consider requesting onsite or virtual vaccine education. 
  • Providing National Healthcare Safety Network (NHSN) support
