Biographies: Mr. Nelson is a former Kidney Transplant Affinity Group Subject Matter Expert. Mr. Bailey is the Associate Director of Patient and Family Engagement for the ESRD NCC. Kim Buettner is the Associate Director of Patient and Family Engagement for the ESRD NCC.

The goals of the Increasing Rates of Patients Dialyzing at Home QIA include:
- Increasing the number of patients training on a home modality within a targeted group of in-center dialysis facilities by 10 percentage points over baseline.
- Improving collaboration between in-center staff and home modality staff.
- Improving knowledge of home modalities for in-center staff to provide needed support to patients transitioning to home training.
- Improving the tracking and transitioning of eligible patients training on a home modality using the 7-Steps to Home Training tracker.
The goal of the Increasing Rates of Patients Dialyzing at Home QIA is to increase the number of patients in training for a home modality by two percentage points from the 2016 baseline in 30% of facilities in the Network service area.
Focus facilities will participate in the Dialyzing at Home QIA through September 30, 2018.

Resources
This Home Dialysis Change Package was created by collecting best practices from highly effective outpatient dialysis facilities and home dialysis programs. It identifies proven roads to success (Primary Drivers) and actionable resources (Secondary Drivers) to support small changes or ideas to help dialysis facilities increase the number of patients that transition to home dialysis. The Network encourages dialysis facilities to review the Home Dialysis Change Package with the IDT during monthly Quality Assessment and Performance Improvement (QAPI) meetings to design a plan for the facility.
