OhioHealth will describe the model of collaborative care for behavioral health that was implemented in selected primary care practices to include implementation considerations, staffing, workflow, and outcome measures. This model uses an embedded behavioral health professional (BHP) collaborating with a primary care provider to care for patients with moderate depression and anxiety with support from psychiatry. The discussion will include how BHP interventions support behavioral change and self-management skills.
This webinar is part of a series of quarterly patient engagement webinars hosted by the Ohio Patient-Centered Primary Care Collaborative (OPCPCC) Center for Provider and Patient Engagement in partnership with Health Services Advisory Group (HSAG). The webinar will begin with a half-hour presentation at 12:15 p.m. ET and will conclude with a 15-minute Q&A session.
