Objectives
- Convene San Francisco, nursing homes, skilled nursing facilities, and home health agencies to engage in conversation and action to reduce readmissions through improving care transitions.
- Explore the importance of advance care planning and best practice interventions.
- Determine readmission challenges to improve care coordination and patient outcomes.
- Discover how to overcome barriers to designing and implementing readmission reduction strategies.

Day of event: registration and breakfast is from 9 a.m. to 9:30 am. event starts at 9:30 a.m.
Parking at the hotel is $9 per hour.
BART directions: Start at Embarcadero Station. Take it to the Cable Car Line (California Line) and travel to the last stop (Civic Center Station), which is at the hotel.
Bus directions: Take bus number 19 or bus number 47 from the BART Station to the Civic Center Station.
Uber/Lyft: Exit at the Civic Center Station.
It is a 15-minute walk from the Civic Center Station to the hotel.
