- To convene healthcare providers and stakeholders to dialogue on interventions to improve transitions across settings.
- To provide resources to improve care coordination and patient safety .
- Review Medicare updates and community readmission data specific to Glendale.

Glendale hospitals, SNFs, HHAs, community stakeholders, CEOs, DONs, RNs, LVNs, Pharmacy, NHAs, and anyone interested in reducing readmissions.
Registration and complimentary breakfast at 8:30 a.m.
Program starts at 9:00 a.m.
Please park in the East Structure. Use Wilson Terrace to enter the driveway to the parking structure. Once inside, park on any level. Take the elevator to the main lobby floor of the East Tower. Please check-in with the Information Desk.
Maximum cost for parking is $6.50. Lost ticket pays maximum of $6.50.
No bills larger that $20 will be accepted. Visa, MasterCard, Discover, and American Express accepted. Checks accepted.
