- Recognize the community's efforts in improving quality and reducing readmissions.

Long Beach Care Coordination Collaborative Quarterly Meeting hospitals, SNFs, HHAs, community stakeholders, CEOs, DONs, RNs, LVNs, Pharmacy, and NH admin.
Registration and complimentary breakfast is from 8:30 a.m. to 9:00 a.m. PT. The event will start promptly at 9 a.m.
