On Lok is providing no-cost, Tomando Control de su Salud workshops for older adults, 60 years and older, living in San Francisco County.
This is the Spanish version of the Chronic Disease Self-Management Program that was designed in Spanish for Spanish- speakers. These programs have proven to help older adults better manage their chronic conditions, improve their quality of life, and lower healthcare costs.
